"...And Nobody Got Hurt" was recorded live at the Pat O'Brien Pavilion in Del Mar, California, USA. The concert took place on the 28th of December in 1991 and was aired to radio the following year on March the 15th. The released versions of ...And Nobody Got Hurt were recorded directly from the radio.
Most of the tracks were not performed in their original key. Songs like "School" and "Smells Like Teen Spirit" were down tuned one semitone. Other tracks, such as "Lithium," were shifted up one semitone.
During the song "Smells Like Teen Spirit," Kurt Cobain sings "Our little tribe has always been...," replacing the word "group," used in the studio recordings, with "tribe."
The track "Lithium" was mislabeled as "Lythium" on most released versions of the album.
